TEE TIME: 9:00 AM

**All teams must be at the course ready to play no later than 8:00 AM.

(one hour prior to tee time).  Lunch will be provided.**

ENTRY FEE: $70 per person or $280 per team. This includes green fees, carts & lunch tickets.  Players who register and pay before April 20, 2026, will receive (2) free mulligans and additional Mulligans can be purchased on the day of play for $5.00 each (cash only - limit of 6 mulligans total per player). Proceeds from the sale of mulligans will be used in a 50/50 drawing.

ENTRY FEES MUST BE RECEIVED NO LATER THAN MAY 15, 2026 (No exceptions)

TEAMS: Each team will need one team captain. Teams shall consist of at least one SMART Local 4 member.

Participation in the tournament will be limited to 18 teams (72 golfers) and will be assigned on a first-come, first-serve basis.  Once 18 teams have registered AND paid, the team registration will be closed.

PRIZES:  The tournament will have one flight with prizes for 1st, 2nd, and 3rd place. On-the-course contests include “Closest to the Pin”, “Longest Drive”, and “Putting Contest”, to name a few.

There will be exciting door prizes. You must be present to win.

Entry fees cannot be refunded after May 15, 2026, as the money will already be committed to prizes and to satisfy the guarantees required by Forest Hill Golf Course.
